We are delighted to welcome you to our practice and are pleased that you chose us to serve your dental needs. We are serious about providing superior dental care and proud of our dedication to our patients. Our goal is to help you feel and look your very best through excellent dental care. Your first appointment will take approximately one hour. In certain cases, it is necessary to take x-rays at our office to better determine the necessary treatment for you. In the case that x-rays are taken, there may be an additional charge. To facilitate being seen just as soon as possible at the time of your appointment, we would appreciate it if you would complete the Patient Information Form. Please remember to bring it with you at the time of your appointment or mail the completed form back to us as soon as possible. Please bring a list of all medications, as well. We also ask that you read, sign and date the financial policy of this office.


The cost of your treatment may vary depending on your individual needs and treatment plan. Though we will be glad to file a dental insurance claim or pre-determination for any necessary future surgery, we do ask that you pay in full for your consultation at the time of the first appointment. We will also be happy to send a claim for any x-rays to your dental insurance company for reimbursement. Remember to bring any Dental Insurance cards that might apply to you or your family to the first appointment. We can then include this information with your records.

Our office is not contracted with any individual insurance company. However, most insurance plans allow you to choose your own provider and will provide benefits accordingly. We understand that it can be difficult to navigate through insurance statements and benefits, so we are more than happy to assist you with this process.

While we do provide short-term payment arrangements for our patients within the office, we also offer Care Credit as a choice for extended payment plans. Click here for more information regarding Care Credit:


In order to assure the most effective use of the doctor's time, it is necessary to inform the office at least two business days in advance before the cancellation of an existing appointment. As we are sure that you are aware, the doctor, hygienists and staff members in the practice strive to meet each and every patient’s needs in a timely manner. We of course understand that emergencies occur and unforeseen inconveniences happen. However, in an effort to have convenient appointments available for you as well as avoid substantial fee increases, our standard policy applies to all visits. If you are unable to make the appointment you have scheduled with us, please notify us at least 48 hours in advance. We would be glad to reschedule the appointment to a more convenient time, if necessary. Thank you very much for your understanding.
